Create an Audit Template

INTRODUCTION:

Audit Templates are created on the Safety Indicators desktop site, first by including some basic information about the Audit, then by adding a Question for each item that’s being assessed.

The Template will then appear on the Mobile App, for those performing the audits. Note that here, the term Audit is used, while some companies use the term Inspections.

OPENING A NEW AUDIT TEMPLATE  

FIRST STEPS

  1. Click Tasks in the left main menu
  2. In the Audits card, click the Settings/Gear Icon at upper right
  3. In the Audit Settings window that appears, click the Audit Templates tab on the left to see the current list of Templates
  4. To open a new Template, click + Create New

FILLING OUT TEMPLATE DETAILS

  1. In the ‘Add Audit Template’ window that appears, type in a Title for the new Template
  2. In the Audit Category field, click the down-arrow, then select the appropriate Category from the drop-down list that appears (required). IF an appropriate category isn't available, click the Plus Sign to add a new one.
  3. Enter a brief Description of this Audit Template
  4. In the Assignee field, click the down-arrow, then select the person who will be performing the Audit. Otherwise, by default, the name of the person creating the Template appears here.
  5. In the Minimum Green Score field, enter the compliance percentage that the Audit results will need to achieve in order to appear in Green in color-coded Insite charts. For example, if you enter 80 here, only Audit results of 80% compliance or better will appear as green. Then add a number in the Minimum Yellow Score field; for example, 50.
  6. In the Report Form field, click on the down-arrow, and select from the drop-down list the type of report that will be generated when a report is run on this Audit
  7. Click the green Save icon

ADDING AUDIT QUESTIONS  

A  Click the Questions tab on the left, so you can begin adding the specific Questions that will be addressed in this Audit

CHOOSING THE TYPE OF QUESTION

B  For the first item, click the down-arrow in the + Create New box, then select the type of question: Text Question; Numeric Question; Yes-No Question; Date Question; Acceptable/Unacceptable Question; Multiple Choice Question; or Multiple Choice - Select Multiple

 

• Yes/No Questions and Acceptable/Unacceptable Questions tend to be the most common types of audit questions.
• Numeric Questions allow you to set a set a range, such as 1 to 5. An answer of ‘4’ on an item’s slider indicates 80% acceptability.
• The Multiple Choice-Select Multiple option allows for more than one answer.

SPECIFYING THE QUESTION

For this example, we’ve chosen an Acceptable/Unacceptable question.

  1. Click the down-arrow in the Category field to select a Category. This is an optional step, but can improve the organization of an audit by grouping questions of the same Category
  2. In the Question field, type in the question itself. For example: “Are the Proper Machine Guards In Place?”
    In the final Template on the Mobile App, this question will be followed by two options: Acceptable and Unacceptable.
  3. In the Question Context field (optional) you can include information that helps the person doing the Audit to understand what exactly they are looking for, and why
  4. When done, click the green Save icon

Note that the Add Audit Question window may have different fields, depending on the type of question.
For example, for a Multiple Choice question, you’ll click a green Plus Sign to add the first ‘choice’ (answer option), then click it again to add each subsequent ‘choice’.

  Click + Create New to choose a question type for the next question in the audit

  Continue until the Audit is complete

FINISHING UP

Note that you can drag to change the order of the Questions, within each category.

When you have completed all the Questions for that Template, click the green disk/Save icon at the top of the Audit Template window, then close out of that window.

LATER REVISIONS
You can return at any time to add new Questions to the Audit Template (+ Create New); edit a Question (double-click it in the list); or Delete a question (select it and click the trash icon). Those revisions will automatically be included the next time that Audit Template is used.

IMPORTING QUESTIONS
To save time, it is possible to import a set of Questions in bulk from a similar Audit Template, then only make minor changes to fit the new Audit Template.
