Incident Subforms

INTRODUCTION:

Basic information about an Incident is always documented in its main Incident Details form. When specific supplemental information is required, a Subform can be filled out after the Incident is created in the system.

 

There are several types of Subforms, depending on what is needed: Injury Claim, Extended Investigations, OSHA Reporting, Property Damages, and others. 

 

Reports may be run based on any of these Subforms, including the ‘300’ and ‘300A’ reports that are required by OSHA at the start of each year.

 

Note that users will only be able to access, view and create Subforms for which they have the proper Permissions. This enables certain information to remain confidential. Those Permissions are determined and conveyed by the System Administrator.
 

 

 

  ACCESSING AN INCIDENT’S EXISTING SUBFORMS  

 

 Go to the List View of Incidents by clicking Safety in the left menu, then clicking the large icon in the Safety Incidents Card

 In the list, double-click the Incident you're interested in

 In the Incident Details window, click Forms in the left menu, to see a current list of Subforms associated with that Incident

 To get more information about a Subform, double-click it in the list to open a Details window

 

 

  CREATING A NEW SUBFORM  

 

 Just as above, go to the List View of Incidents by clicking Safety in the left menu, then clicking the large icon in the Safety Incidents Card

 Double-click the Incident you want to create a subform for


A  In the Incident Details window, select Forms in the left menu

B  At the top, click the down-arrow next to + Create New, then select the type of Subform you want to add

 

 

 

  COMPLETING THE VARIOUS SUBFORMS  

 

When the new Subform opens, you will notice that certain fields are already filled, based on the information provided in the main Incident Details form. Typically, the other fields are optional; fill them in as needed, and as shown below.

OSHA Reportings Subform

This Subform is typically associated with Incidents deemed Recordable.

 

A  If the employee has lost work time, check the Lost Time checkbox, and click on the calendar icons to select the first day of absence, and the day the employee returned. Then enter the total number of Lost Work Days (LWD)

B  If work-related restrictions have been placed on an injured employee, check the Restriction checkbox, and click on the calendar icons to select the first day of restricted activity, and the day they returned to full duty. Then enter the total number of Restricted Work Days (RWD)

 


 

Note that there is already a checkmark in the ‘Recordable’ check box. By clicking Save with that checkmark in place, you are tagging this Incident as Recordable, and the new Subform will now be attached to that Incident.

Injury Claim Subform
This Subform is typically associated with Workers Comp cases.


The focus of here are the medical and related cost to the company.

 

A  Click the down-arrow in the Insurer Claim Type field to select a 'Claim Type' option

B  Click the down-arrow in the Est Claim Cost field to select a cost range

C  Type in the total amount of the cost to the company

Extended Investigations Subform

This Subform is typically associated with factors that contributed to the Incident.


A  In the second section, use the down-arrow to select an answer from the available standard options. Those who have proper Permissions may click the Plus Sign in each field to add their own explanations.

 

 

Information captured in this form will be used as data points in reports and charts, to identify the most common contributing factors in company Incidents.

Property Damage Subform
This Subform is typically associated with damage to vehicles or equipment.


The focus here is the out-of-pocket cost to the company.


A  Enter the Estimated Cost of Repair in the provided field
B  If a repair estimate is available, you can attach it to this Subform. Click Choose File and navigate to that document OR drag the document to the provided box

 

 

Note that a separate Property Damage Subform must be created for each item that has been damage

  In all cases, when you're done, click Save at the bottom
