Tag an Incident as Recordable

OVERVIEW:

Injuries that are defined as OSHA Reportable/Recordable must be tagged with an appropriately completed OSHA Reporting subform. Creation of the OSHA Reporting subform will enable the injuries to A) show up properly on charts and appropriate rates metrics and B) will show up on any OSHA logs and reports. This guide shows the steps to do this for those with the proper permission to the OSHA Reporting subform.

 

 

  FINDING THE INCIDENT REPORT  

 

STEP 1

•  Select Safety in the left main menu

•  In the Safety Incidents Card, click on the large icon

STEP 2
A  Find the Incident Report you need in the list and double-click it

B  OR, enter the Case Number/ID in the Search box, then hit Enter/Return on your keyboard. Double-click on the Incident Report listing that comes up

 

C  OR, click the down-arrow next to one of the Filters, to specify parameters that will narrow down the list. Then double-click on the appropriate Incident Report

  ACCESSING THE PROPER FORM  

STEP 1
A  In the Incident Details window, select Forms in the left menu
B  Click the down-arrow in the + Create New button, then select OSHA Reporting

  ENTERING THE REQUIRED INFORMATION  

 

STEP 1

A  In the ‘Add OSHA Reporting’ window, scroll down to Lost Time on the left.

IF the employee has (or is expected to have) lost work time due to their injury, check Lost Time.   

Then click the calendar icons to specify the Date Lost Time Began and Date Returned from Lost Time. 

The number of LWD (Lost Work Days) automatically appears in the Total LWD field.

 

B  IF the employee is on (or is expected to be on) Restricted Activity due to their injury, check Restriction

Then click the calendar icons to specify the Date Began Restricted Activity and Date Returned to Full Duty  

The number of RWD (Restricted Work Days) automatically appears in the Total RWD field.

 

•   Optionally, it may be helpful to add other relevant information to this form

•   You'll see that the checkbox for 'Recordable' has been automatically checked

 

D  Click the green Save button at the bottom. You’re done.

  • This recordable incident can now be represented in OSHA Reports, such as forms 300, 300A and 301.
  • This recordable incident will also be included in the Recordable Injury Matrix found within InSites, for those with the necessary Permissions to access that data.
