Understanding Audit Issue Categories & Subcategories

Audit Issue Categories and Subcategories allow your organization to classify findings within an audit in a structured, consistent way. While your Audit Template Categories (e.g., Safety, Environmental) define what is being audited, the Issue Categories and Subcategories describe what was found during that audit — such as a non-conformance, observation, or improvement opportunity. 

How They Work

  1. Issue Categories represent the primary type of issue or finding.

  2. Issue Subcategories further define the specific nature of the problem.

When a finding is logged during an audit, users can select both fields to ensure findings are grouped consistently across all audits.

Examples

Audit Template Category Issue Category Issue Subcategory Description
Safety Audit Personal Protective Equipment (PPE) PPE Not Worn Employee failed to wear required gloves or eye protection.
Environmental Inspection Waste Management Spill Containment Inadequate Secondary containment not in place for chemical drums.
Site Housekeeping Work Environment Trip or Slip Hazard Cables and hoses running through walkways without protection.

Recommended Use

  • Before Auditing: Ensure your Issue Categories and Subcategories are aligned with your internal standards or risk areas.

  • During Auditing: Select the most appropriate category/subcategory that relates to the findings.

  • After Auditing: Use these categories to generate consistent reports and identify trends across locations or time periods.

If needed, your Admin can customize these categories to reflect your organization’s processes from within Audit Settings.
